Job Title: Bilingual Case Manager

Devereux Advanced Behavioral Health is seeking a highly skilled and compassionate Bilingual Case Manager to join our team. As a key member of our staff, you will play a vital role in providing exceptional care and support to unaccompanied children in our shelter program.

Job Summary:

The Bilingual Case Manager will be responsible for developing and implementing individualized treatment plans, coordinating with external providers, and ensuring the seamless delivery of services to our clients. This is a full-time position that requires a strong commitment to the health and well-being of our children.

Responsibilities:
  • Develop and implement individualized treatment plans for children and families
  • Coordinate with external providers to ensure comprehensive care
  • Provide emotional support and guidance to children and families
  • Facilitate communication between children, families, and external providers
  • Monitor and evaluate treatment plans to ensure progress and compliance
Requirements:
  • Bachelor's degree in a behavioral science or related field
  • Bilingual in English and Spanish
  • Excellent communication and interpersonal skills
  • Ability to work in a fast-paced environment
  • Valid Connecticut driver's license
