Make a Difference in Home Care
At Professional Case Management, we are seeking a skilled Senior Case Manager to join our team. As a Senior Case Manager, you will be responsible for providing direct care and case management to assigned clients, collaborating with regional directors and other personnel to identify and correct issues, and implementing and evaluating care provided.
Key Responsibilities:
- Provide direct care and case management to assigned clients
- Collaborate with regional directors and other personnel to identify and correct issues
- Implement and evaluate care provided
- Participate in interviewing, selection, and ongoing evaluation of clinical personnel
- Review and evaluate clinical documentation for accuracy and completeness
- Participate in all Agency performance improvement initiatives
Requirements:
- Graduate of a state-approved school of professional registered nursing
- BSN preferred
- Current, unrestricted RN license in the state(s) of practice
- Minimum of two (2) years nursing experience, including one (1) year in home care or a closely related field
- One (1) year of supervisory and/or case management experience preferred
- Current CPR certification
What We Offer:
- $44.00 hourly plus shift differentials, where applicable
- Bonus eligible
