Physical Therapist

Amergis is seeking a skilled Physical Therapist to join our team. As a Physical Therapist, you will be responsible for providing high-quality physical therapy services to our clients.
Key Responsibilities- Perform student evaluations and provide therapy services in accordance with the plan of care developed for each student.
- Collaborate with the physician and other members of the healthcare team to modify the plan of care as necessary.
- Participate in developing the plan of care in collaboration with the physician and other members of the healthcare team.
- Assess and evaluate the physical needs of students and provide a complete physical assessment and history of current and previous illnesses.
- Provide physical therapy treatment and establish a physical therapy plan of care.
- Instruct students, families, and/or caregivers in the use of and care of therapeutic appliances.
- Report student reactions to treatment or changes in condition to the physician and/or Building Nurse and Amergis Clinical designee.
- Initiate physical therapy programs and instruct, consult, and/or advise other personnel and/or family/caregiver regarding certain phases of physical therapy.
- Train students in the use of prosthetic devices and other assistive devices.
- Use appropriate equipment to meet student goals and clean and maintain equipment according to manufacturer's guidelines.
- Prepare and submit clinical and progress summaries based on the attainment and progress of goals, and responses to treatment or changes in condition.
- Participate in peer consultation and supervision of Physical Therapy Assistants according to company policies/procedures and State regulations.
- Participate in quarterly medical record reviews and care coordination with the company and other personnel caring for the student.
- Provide and submit accurate and timely documentation in accordance with the plan of care and company policy and procedures.
- Current licensure as a Physical Therapist in the state of practice.
- Graduate of an accredited school of Physical Therapy.
- One (1) year of prior professional Physical Therapy experience preferred.
- Compliance with all relevant professional standards of practice.
- Participation and completion of Amergis' Competency program when applicable.
- Current CPR if applicable.
- TB Questionnaire, PPD or chest x-ray if applicable.
- Current Health certificate (per contract or state regulation).
- Must meet all federal, state, and local requirements.
- Successful completion of new hire training as applicable to job site.
- Understanding of patient confidentiality and HIPAA requirements.
- Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required.
- Computer proficiency required.
- Must be at least 18 years of age.
