Job Title: Physical Therapist

Amergis is seeking a skilled Physical Therapist to join our team on a part-time basis, working 2 days a week to support our Middle and High School students.

Job Summary:

The Physical Therapist will be responsible for performing student evaluations, providing and documenting therapy services in accordance with the plan of care developed for each student and per physician's orders when applicable.

Key Responsibilities:
  • Perform student reassessments and regularly reevaluate students for rehabilitation needs, working in collaboration with the physician and other members of the healthcare team to modify the plan of care as appropriate.
  • Participate in developing the plan of care in collaboration with the physician and other members of the healthcare team.
  • Assess and evaluate therapeutic, rehabilitative, and functional status, as applicable.
  • Provide physical therapy treatment and establish a physical therapy plan of care.
  • Instruct students, families, and/or caregivers in the use of and care of therapeutic appliances.
  • Determine priority needs for physical therapy.
  • Report students' reactions to treatment or changes in condition to the physician and/or Building Nurse and Amergis Clinical designee.
  • Initiate physical therapy programs and instruct, consult, and/or advise other personnel and/or family/caregiver regarding certain phases of physical therapy.
  • Train students in the use of prosthetic devices and other assistive devices, ensuring equipment used in the course of student treatment is in good working order.
  • Use appropriate equipment to meet student goals, clean, and maintain equipment according to manufacturer's guidelines.
  • Prepare and submit clinical and progress summaries based on the attainment and progress of goals, and responses to treatment or changes in condition.
  • Participate in peer consultation and quarterly medical record reviews, as applicable.
  • Supervise Physical Therapy Assistants according to the company's policies/procedures and State regulations.
  • Participate in care coordination with the company and other personnel caring for the student to ensure that their efforts effectively complement each other and support the objectives outlined in the plan of care.
  • Provide/submits accurate and timely documentation in accordance with the plan of care and company policy and procedures.
  • Comply with applicable local, state, and federal laws and regulations regarding professional standards and practice standards.
Requirements:
  • Current licensure as a Physical Therapist in the state of practice.
  • Graduate of an accredited school of Physical Therapy.
  • One (1) year of prior professional Physical Therapy experience preferred.
  • Comply with all relevant professional standards of practice.
  • Participation and completion of Amergis' Competency program, when applicable.
  • Current CPR, if applicable.
  • TB Questionnaire, PPD, or chest x-ray, if applicable.
  • Current Health certificate, per contract or state regulation.
  • Must meet all federal, state, and local requirements.
  • Successful completion of new hire training, as applicable to the job site.
  • Understand patient confidentiality and HIPAA requirements.
  • Ability to effectively elicit/provide information to and from appropriate individuals, including supervisors, co-workers, clients, via strong communication skills; proficiency in the English language is required.
  • Computer proficiency required.
  • Must be at least 18 years of age.
