About the Role:

The Housing and Employment Resource Specialist will work directly with Transitional Age Youth to help meet their service and resource needs, including housing, educational services, job training, transportation, and other support services.

Key Responsibilities:

  • Provide intensive case management support to Transitional Age Youth.
  • Develop and implement individualized service plans to address housing, educational, and employment needs.
  • Collaborate with community-based organizations to provide access to resources and services.
  • Provide peer support and independent living skills development to Transitional Age Youth.
  • Conduct individual and family counseling sessions as needed.
  • Provide mental health and medication management support to Transitional Age Youth.

Requirements:

  • Bachelor's degree in a social science field.
  • Two years of experience performing non-professional, consumer-directed services in behavioral health, healthcare, or social services.
  • Valid California driver's license.
