Therapist - Service Coordinator

North Suffolk Mental Health Association is seeking a skilled Therapist - Service Coordinator to join our Early Intervention Team. As a passionate advocate for children and families, you will play a vital role in providing developmental and expressive therapy services to infants and toddlers with developmental delays or at risk for delays.

Key Responsibilities:

  • Conduct EI screenings, intakes, evaluations, and assessments to identify client needs and develop individualized plans.
  • Provide direct child developmental and family support, including case management and coordination of intervention services.
  • Collaborate with client families and the EI team to identify concerns, priorities, and resources.
  • Refer clients to specialized therapies and case management services as needed.
  • Coordinate intervention services with medical, state, and health providers.
  • Assist in developing transition plans for child and family clients.

Requirements:

  • Master's degree in Expressive Therapy or a related field.
  • Licensure as a LICSW or LMHC is preferred.
  • Experience in infancy/early childhood and special education is preferred.
  • Early Intervention certification with DPH is required.
  • Driver's license and access to a reliable vehicle are necessary for off-site services.
