Key Responsibilities- Engage with members and their caregivers to assess health needs and develop personalized care plans.
- Conduct comprehensive assessments, including physical and mental health histories, to identify barriers to care.
- Coordinate services across physical health, behavioral health, and social support systems.
- Facilitate medication management and adherence through regular follow-ups and reconciliations.
- Serve as a primary contact for members, advocating for their needs within the care management program.
- Document progress and adjust care plans as necessary to ensure optimal health outcomes.
The ideal candidate will possess:
- A Bachelor’s Degree in Nursing.
- At least 3 years of clinical experience working with medically fragile pediatric patients.
- A minimum of 3 years of experience in case management, preferably in a telephonic capacity.
- Proficiency in Microsoft Office and electronic medical record systems.
- A valid driver’s license and car insurance.
