Overview & Responsibilities:

As a Licensed Practical Nurse, you will provide direct nursing care to residents while overseeing the care delivered by medication care specialists. Your responsibilities will include administering medications, performing treatments and procedures, obtaining specimens as necessary, and completing required documentation. The essential duties and responsibilities outlined below represent the core functions of this role:

  • Document resident status in accordance with policy.
  • Supervise assigned team members while delivering nursing care to residents.
  • Assist with the onboarding of new residents.
  • Support residents with Activities of Daily Living (ADL) and promote their independence.
  • Respond promptly and courteously to resident calls.
  • Identify medical and behavioral changes in residents and develop care plans accordingly.
  • Report daily to the Community Wellness Director regarding community conditions and resident needs.
  • Administer nursing care as per physician orders and established standards.
  • Coordinate nursing care with other disciplines to ensure comprehensive service delivery.
  • Assist physicians during rounds and examinations.
  • Monitor resident health status and notify the attending physician of any changes.
  • Accurately administer prescribed medications and provide first aid as needed.
  • Lead and supervise assigned team members in meeting resident needs.
  • Assess and adjust care plans based on ongoing evaluations.
  • Orient residents and team members to emergency procedures.
  • Complete incident reports and follow up as required.
  • Ensure all resident records are accurate and complete.
  • Maintain a safe and sanitary environment for residents and staff.
  • Deliver exceptional customer service in all interactions.
  • Adhere to all community policies and procedures.
  • Perform incidental housekeeping and maintenance tasks as necessary.
  • Complete other related duties as assigned.

Qualifications:

  • Current nursing license in the state of employment.
  • One year of nursing experience, preferably with dementia patients.
  • Strong interpersonal and teamwork skills.
  • Ability to process written information and complete documentation accurately.
  • Demonstrated compassion and integrity in working with older adults.
  • Required to receive the annual Flu vaccine, with exceptions for religious or medical reasons.
