Job Summary

We are seeking a highly organized and detail-oriented Discharge Planning Coordinator to join our team at Yale New Haven Health. As a key member of our care management team, you will be responsible for ensuring a smooth transition of care for our patients from hospital to home or other healthcare settings.

Key Responsibilities:

  • Coordinate with healthcare teams to develop and implement discharge plans that meet the unique needs of each patient.
  • Communicate with patients, families, and healthcare providers to ensure a seamless transition of care.
  • Identify and address potential barriers to discharge, such as insurance or equipment needs.
  • Collaborate with care management staff to prioritize and manage a high-volume caseload.
  • Maintain accurate and up-to-date records of patient information and discharge plans.

Requirements:

  • Associate's or Bachelor's degree in a healthcare-related field.
  • 2-3 years of experience in a healthcare environment, preferably in discharge planning or care management.
  • Excellent communication and organizational skills, with the ability to work independently and as part of a team.
  • Proficiency in electronic medical records and other healthcare software.
