Job Summary

We are seeking a skilled Clinical Care Coordinator to join our team at New London Hospital. As a key member of our healthcare team, you will be responsible for managing patient care outcomes and providing leadership for the management of patient care and resulting outcomes.

Key Responsibilities
  • Determine patient needs and resources within 1-2 business days of admission and initiate discharge planning process.
  • Maintain communication with UM coordinator regarding assessments, outcomes, and projected lengths of stay.
  • Collaborate with the physician/multidisciplinary team to review the plan and progress of care, evaluate the effectiveness of interventions, and recommend modification(s) to the plan as necessary to meet identified patient care goals.
  • Coordinate transfers to appropriate facilities when applicable and secure authorization for needed additional patient services.
  • Document case management assessments and interventions as appropriate, adhering to current standards and policies.
  • Support the job functions of other members of the OCM team as needed.
Requirements
  • 3 years of experience in a clinical or community care setting.
  • Experience at a Critical Access Hospital preferred.
  • Current BLS certification or ability to obtain within 30 days of hire.
  • Graduate of an accredited school of nursing and licensed to work as an RN in the State of NH.
