Discharge Planning Coordinator

Overview
At Yale New Haven Health, we are seeking a skilled Discharge Planning Coordinator to join our team. As a key member of our healthcare team, you will be responsible for ensuring a smooth transition of care for our patients from initial contact through discharge. Your proactive approach and collaboration with our Care Management staff and healthcare team will be crucial in developing solutions to complex discharge processes.
Key Responsibilities:
- Coordinate placement of hospitalized patients with extended care facilities, home care, or other community-based agencies.
- Facilitate delivery of equipment and supplies as directed by Care Coordinator or Social Work partner.
- Communicate with nursing homes, agencies, and vendors to achieve discharge plans.
- Monitor and prioritize discharge plans for each patient, ensuring timely and safe discharge.
- Document actions and maintain accurate patient records.
Requirements:
- Associate Degree and/or Bachelor of Science Degree in Business Administration, Human Services, Health Administration, or other healthcare-related field.
- Two to three years of experience in a healthcare environment.
- Excellent communication and organizational skills.
