Job Summary

We are seeking a highly skilled Registered Nurse to join our team at Southeast Primary Care Partners. As a key member of our clinical staff, you will provide exceptional patient care and support functions in a fast-paced primary care setting.

Key Responsibilities
  • Assist Providers and clinical staff in identifying high-risk patients and implementing band stratification in electronic medical records.
  • Conduct evaluations and patient education nurse visits for chronic disease states, including clear instruction on treatment, medications, and self-management goals.
  • Triage incoming calls and perform outbound calls regarding band stratification allocation.
  • Assist in reviewing patient supportive care at home, social factors, and concerns, including assessment of durable medical equipment and home health resources.
  • Communicate effectively with hospital and skilled nursing facilities regarding inpatients and update current patient information and status.
  • Perform clinic patient wound care on stable conditions and assess and document appropriately.
  • Work collaboratively with multidisciplinary teams and individuals to provide high-quality patient care.
Requirements
  • Registered Nurse with a current license in good standing to practice in the state of employment.
  • Current CPR/BLS certification.
  • Ability to work efficiently with electronic medical records and effectively use proper grammar, spelling, and medical terminology.
  • Two years of related RN experience in a physician's office, clinical, or hospital setting.
  • Current IV certification and effective listening and communication skills.
Preferred Qualifications
  • Three or more years of related RN experience in a physician's office, clinical, or hospital setting.
  • Knowledge of managed care and referral processes.
  • Experience related to patient education and proficient knowledge of chronic diseases, especially COPD/asthma, CHF, IHD, and diabetes.
Physical and Mental Requirements
  • Ability to lift up to 50 pounds.
  • Ability to push or pull heavy objects using up to 50 pounds of force.
  • Ability to sit for extended periods and stand for extended periods.
  • Ability to use fine motor skills to operate office equipment and/or machinery.
  • Ability to receive and comprehend instructions verbally and/or in writing.
