Job DescriptionThe successful candidate will be responsible for:
- Providing person-centered support to individuals with IDD, promoting independence, dignity, and self-advocacy.
- Developing and implementing individualized support plans that cater to the unique needs and preferences of each consumer.
- Collaborating with clinical staff to ensure seamless coordination of services and promote healthy behaviors.
- Assisting consumers with daily living tasks, such as meal preparation, toileting, and hygiene, as needed.
- Supporting community integration activities, including medical appointments, social security access, and other essential services.
- Completing required documentation and reporting to ensure compliance with regulatory requirements.
To succeed in this role, you will need:
- A High School diploma or GED; an Associate's or Bachelor's degree is preferred.
- Basic computer and internet literacy, with proficiency in applications like email, time recording, and data entry.
- Prior experience in the human services field, preferably in disability support or a related field.
- A valid driver's license, with a clean driving record.
- Subject to employment screenings, including FBI clearance (if applicable).
We offer a competitive salary, estimated at $45,000 - $60,000 per annum, based on experience and location. Additionally, we provide:
