Job Description

The successful candidate will be responsible for:

  • Providing person-centered support to individuals with IDD, promoting independence, dignity, and self-advocacy.
  • Developing and implementing individualized support plans that cater to the unique needs and preferences of each consumer.
  • Collaborating with clinical staff to ensure seamless coordination of services and promote healthy behaviors.
  • Assisting consumers with daily living tasks, such as meal preparation, toileting, and hygiene, as needed.
  • Supporting community integration activities, including medical appointments, social security access, and other essential services.
  • Completing required documentation and reporting to ensure compliance with regulatory requirements.
Requirements

To succeed in this role, you will need:

  • A High School diploma or GED; an Associate's or Bachelor's degree is preferred.
  • Basic computer and internet literacy, with proficiency in applications like email, time recording, and data entry.
  • Prior experience in the human services field, preferably in disability support or a related field.
  • A valid driver's license, with a clean driving record.
  • Subject to employment screenings, including FBI clearance (if applicable).
