Job Summary

We are seeking a highly skilled Care Manager to join our team at Community Care of North Carolina. The successful candidate will be responsible for providing effective care management services to enrolled populations, working closely with members, providers, and community partners to ensure coordination of services.

Key Responsibilities
  • Conduct comprehensive assessments of members' health care needs, considering medical, biopsychosocial, behavioral, spiritual, and cultural factors.
  • Develop and implement personalized care plans in collaboration with members, caregivers, and healthcare providers.
  • Provide education and support to members and their families to promote optimal self-management and health outcomes.
  • Monitor and evaluate the effectiveness of interventions, making adjustments as needed to achieve desired outcomes.
  • Collaborate with multidisciplinary teams to facilitate achievement of treatment goals and promote quality care.
  • Facilitate referrals to community-based services and agencies, ensuring seamless coordination of care.
Requirements
  • Registered Nurse (RN) or Master's degree in Social Work (MSW) from an accredited institution.
  • Active, unrestricted RN license or LCSW in North Carolina.
  • Minimum 2 years' experience in care management, community-based nursing, or social work.
  • CCM certification preferred, with eligibility to obtain within 1 year.
  • Strong communication and interpersonal skills, with ability to work independently and as part of a team.
  • Knowledge of government, private sector, and community resources, as well as case management principles and federal/state regulations.
Working Conditions

The job environment is primarily an office or home environment, with multiple contacts with members, providers, and community partners. Travel will be required within the region and/or the State.
