Job Summary

The Ambulatory Care Nurse (ACN) is a Registered Nurse responsible for providing nursing care management services to assigned populations. This role utilizes a collaborative approach to anticipate, integrate, and coordinate patient care plans, maximizing the ability to remain in a community setting.

Key Responsibilities
  • Identify at-risk patient populations who require care management services in collaboration with physicians.
  • Serve as a patient/family advocate and act as a communication link between the physician, patient, family, and interdisciplinary healthcare team, as well as community resources.
  • Support the mission, values, and management of patient care services, actively incorporating ANA Scope and Standards of Nursing Practice, North Carolina Board of Nursing Practice Act, WakeMed's Division of Nursing Professional Practice model, and WakeMed's Patient & Family Centered Care (PFCC) model.
Requirements
  • 2 Years Nursing-Related Area or No Experience Required With Preferred Education.
  • Diploma Nursing or Associate's Degree Nursing Required - And Bachelor's Degree Nursing Preferred.
  • Registered Nurse Required.
Work Schedule

8 am to 4:30 pm, Monday through Friday, with no weekend requirements.
