The Patient Care Management Supervisor plays a crucial role in overseeing patients engaged in complex care management (CCM) services, aiming to enhance care coordination and health outcomes. This is achieved through the formulation and execution of patient-centered care strategies and collaboration with a multidisciplinary team of healthcare professionals.

The individual in this role must exhibit exemplary communication and interpersonal skills, demonstrating diplomacy and sensitivity while navigating a dynamic, fast-paced healthcare environment characterized by numerous distractions and shifting priorities.


Educational Requirements:

A Bachelor’s Degree is required; a Master’s Degree is preferred.

Experience:

A minimum of 2 years of experience in Case Management is required, with supervisory experience being advantageous.

Licenses:

An active RN license is mandatory.

Certifications:

CCM or ACM certification is preferred, with a requirement to obtain within one year of employment.

Knowledge, Skills, & Abilities:

Active RN license, with CCM or ACM certification preferred; 2 years of recent Case Management experience; flexibility to work with a diverse patient population.

Key Responsibilities:

  • Complex Care Management: Identify patients at the highest risk for health deterioration, sentinel events, and/or adverse outcomes.
  • Collaboration: Work alongside other care team members, including but not limited to the Primary Care Provider, Clinical Pharmacist, Nutritionist, Behavioral Health, and Case Management teams to ensure seamless coordination and continuity of care.
  • Assessment: Conduct comprehensive evaluations of new care management patients and collaborate with the patient, caregivers, and care team to develop individualized care plans that reflect each patient’s treatment goals, strengths, and challenges.
  • Coordination: Facilitate connections with external organizations and community partners, such as VNA agencies, caregiver programs, DME providers, social service agencies, and external care managers.
  • Transition Management: Oversee patient transitions to and from external care facilities or providers to mitigate potential complications.
  • Patient Empowerment: Utilize motivational interviewing techniques to foster patient empowerment, self-management, and active participation in care-related decision-making.
  • Advocacy: Serve as an advocate for patients to ensure they receive care that is safe, effective, patient-centered, timely, efficient, and equitable.
  • Resource Sharing: Compile and disseminate community-based resources to other care team members to enhance overall patient care.
Professional Development:

Maintain awareness of personal professional development needs and communicate these by establishing annual performance goals.

Additional Duties:

Perform other responsibilities as assigned.
