About the Role

We are seeking a highly skilled Field Care Coordinator to join our High Risk team at One Medical. As a Field Care Coordinator, you will play a critical role in supporting our patients with care outside of the traditional clinic setting, providing essential services in their homes and via telehealth.

Key Responsibilities:

  • Facilitate telehealth visits in patients' homes, ensuring seamless communication between patients, providers, and care teams.
  • Conduct standard patient assessments and documentation to facilitate complete visits by treating providers.
  • Perform clinical duties, including providing best-in-class telehealth setup services, conducting health status screenings, and collecting non-blood specimens.
  • Coordinate care with patients, families, and outside care teams, ensuring comprehensive support and navigation of community resources.
  • Teach patients and families to use technology to connect with their care teams, promoting independence and self-management.

Requirements:

  • At least 1 year of experience as a Medical Assistant, with a preference for experience in home-based healthcare services or senior living care settings.
  • Graduation from a Medical Assistant Program accredited by the Commission on Accreditation of Allied Health Education Programs (CAAHEP) or the Accrediting Bureau of Health Education Schools (ABHES).
  • Certified Medical Assistant (MA) and Basic Life Support (BLS) certification, with the ability to obtain certification within 6 months of hire.
  • Strong written and verbal communication skills, with proficiency in computer technology and video platforms.
  • Ability to travel throughout the Phoenix area, with reliable transportation and a valid driver's license.
