Senior Care Coordinator

We are seeking a compassionate and organized Resident Care Coordinator/LPN to join our team at Shadow Mountain Memory Care. The ideal candidate will have a strong background in nursing and excellent communication skills.
Key Responsibilities:
- Supervise and coordinate the daily healthcare needs of residents.
- Review and document daily communication logs, new orders, and medication delivery systems.
- Train and in-service staff on resident care and medication procedures.
- Communicate with outside agencies and medical providers to ensure seamless care.
- Review and respond to incident reports and alert charting.
- Ensure cleanliness and organization of medication rooms and carts.
- Assist residents with meals and coordinate service plan reviews.
- Participate in staffing on-call duties and maintain adequate staffing levels.
Requirements:
- Current active license as a Practical Nurse in the State of Arizona.
- Previous work with the elderly population and experience as a medication assistant are desirable.
- CPR Certification is required.
- Ability to lift 40-50 pounds regularly during work hours.
