Job Description:

The Birth to Five Care Coordinator plays a crucial role in the treatment of children aged 0-5 years. This hands-on position involves advocating and collaborating with patients and their caregivers to ensure the developmental, academic, and social/emotional needs of young children are addressed.

The Birth to Five Care Coordinator supports patients in accessing resources and tools needed to reach specified goals related to behavior modification, parent education, and resource acquisition. This individual understands the value and challenges of navigating the systems of care available to our community, specifically as it relates to Birth to Five supportive services.

The Birth to Five Care Coordinator may work closely with the Department of Child Safety and complete a protracted assessment to inform goals and treatment planning for the child and caregivers involved. Clinical assessments and gathering information about social/emotional needs, trauma history, and other concerns are also part of this role.

The Birth to Five Care Coordinator will receive specific training on the birth to five demographics, including the utilization of developmental screeners, parent-child observational assessments, and evidence-based intervention models to ensure that the services provided are best practice for the patient. Services may be provided in office, in the home, in the school, or in the community, as well as virtually.

The Birth to Five Care Coordinator may have up to 35 active patients on their caseloads. Benefits include:

Job Qualifications:
  • Behavioral Health Technician II (BHT II)
  • 18 years of age or older
  • Bachelor's or master's degree in behavioral health (B.H.) related field
  • Clinical Intern in bachelor's or master's degree behavioral health program in good standing
  • Bachelor's or master's degree in a non-BH related field AND 1 year of BH work experience
  • Associate degree AND 2 years of BH work experience
  • High School diploma and 4 years of experience in a healthcare setting (1 of the 4 years must be direct patient contact)
  • Must have the skills and knowledge necessary to provide the behavioral health services that the agency is authorized to provide and to meet the unique needs of consumer populations served by the agency, including children and meet requirements Behavioral Health Technician II (BHTII)
  • Strong verbal, written, and communications skills
  • Knowledge of Windows environment and/or Electronic Health Records Systems
  • Ability to provide culturally sensitive services
  • Must have or be able to obtain a valid Fingerprint Clearance Card
  • Must possess reliable transportation, valid driver's license, and proof of auto insurance
  • Must maintain current BLS certification
  • Covid 19 Vaccination or Qualified Exemption (Religious or Medical)
  • TB Screening Record and PPD/QuantiFERON/Chest x-ray as applicable upon hire
  • Vaccinations/Titer's Test/Declination Form: Flu, Hep B, MMR, Tdap, Varicella
