Job Summary

We are seeking a skilled and experienced Licensed Vocational Nurse (LVN) to join our team as a Hospice Quality Assurance Nurse. This role is responsible for ensuring the highest quality of care for our patients and their families, while also maintaining compliance with regulatory and accreditation standards.

Key Responsibilities
  • Monitor and evaluate the quality of care provided to patients, including chart reviews and data collection.
  • Develop and implement quality improvement initiatives to enhance patient care and outcomes.
  • Collaborate with nursing supervisors to ensure accurate and complete documentation of patient care.
  • Act as a resource to staff on quality and safety issues, and provide education and training as needed.
  • Conduct regular audits and reviews to ensure compliance with regulatory and accreditation standards.
  • Develop and maintain aggregate records of employee orientation and annual competency requirements.
  • Monitor organization compliance with regulatory and accreditation standards.
  • Act as a clinical resource to staff and provide guidance on standards, policies, and regulations.
Requirements
  • Licensed Vocational Nurse (LVN) with a minimum of 2 years of experience in hospice or home care nursing.
  • Excellent understanding and use of Excel, PowerPoint, and other data-base programs.
  • Three (3) to five (5) years of clinical nursing experience, with an emphasis in home care/hospice nursing preferred.
  • Two (2) years of experience in Quality Assurance preferred.
  • Excellent understanding of performance improvement, quality assessment, and utilization management.
  • Self-directed and able to work with minimal supervision.
  • Excellent coordination and communication skills.
  • Bilingual is highly desirable but not necessary.
Working Conditions

This is a full-time position that requires the ability to work in a fast-paced environment with minimal supervision. The successful candidate will be required to maintain confidentiality and handle sensitive information with discretion.
