Job Summary

Topanga Terrace is seeking a skilled Licensed Vocational Nurse to join our team. As a key member of our healthcare staff, you will be responsible for providing high-quality patient care and services in a subacute care facility setting.

Key Responsibilities
  • Coordinate and Evaluate Patient Care: Develop and implement individualized care plans to meet the unique needs of each patient.
  • Supervise Resident Care Activities: Oversee the delivery of care and services provided by Certified Nursing Assistants, ensuring adherence to established standards and protocols.
  • Complete Medical Forms and Reports: Accurately and thoroughly document patient information, medical histories, and treatment plans.
  • Participate in Quality Assurance: Collaborate with the facility's QA program to identify areas for improvement and implement changes to enhance patient care and services.
  • Ensure Reference Materials are Maintained: Organize and maintain reference materials, including regulations, policies, and procedures, at the nurses' station.
  • Admit, Transfer, and Discharge Patients: Ensure a smooth transition of care for patients, including admission, transfer, and discharge processes.
  • Investigate Incidents and Accidents: Conduct thorough investigations and complete incident/accident reports as required.
  • Interpret Facility Policies and Procedures: Communicate facility policies and procedures to staff, patients, visitors, and government agencies as needed.
  • Attend Facility Meetings: Participate in various facility meetings, including care conferences, stand-up meetings, and nurses' meetings.
  • Process Telephone Orders: Receive and process telephone orders from physicians, updating patient care plans as required.
  • Report Care-Ordered but Not Delivered: Notify the Director of Nursing of any care ordered but not delivered, medication errors, charting errors, diet changes, and other discrepancies.
  • Complete Medication and Treatment Passes: Administer medications and treatments in a timely and accurate manner, adhering to state and federal regulations and facility policies.
  • Review Medication and Treatment Administration Records: Periodically review records for completeness and accuracy, reporting any discrepancies to the Director of Nursing.
  • Inform Nursing Staff of New Admissions: Notify nursing staff of new admissions, including expected arrival times and room assignments.
  • Make Daily Resident Rounds: Conduct daily rounds to observe and evaluate patients' physical and emotional status.
  • Meet with Residents and Families: Meet with patients and families as needed to discuss care plans and address concerns.
  • Notify Attending Physicians and Families: Inform attending physicians, families, and the interdisciplinary team of any incidents, accidents, or changes in patient condition.
  • Consult with Attending Physicians: Collaborate with attending physicians as needed for treatments, medications, and other patient care matters.
  • Participate in Infection Control: Participate in the facility's infection control program to monitor communicable and infectious diseases among patients and staff.
  • Report Hazardous Conditions or Equipment: Identify and report any hazardous conditions or equipment to the Director of Nursing.
  • Partner with Director of Nursing: Collaborate with the Director of Nursing to ensure staff coverage requirements are met.
  • Give and Take Report: Provide and receive report upon beginning and ending of shift.
Requirements
  • Nursing Degree: Possess a nursing degree from an accredited school or college.
  • Valid LVN License: Hold a valid LVN license in good standing.
