We offer a comprehensive range of evidence-based services, including mental health and substance abuse treatment, housing support, vocational training, job placement, healthcare, education, and creative arts therapies.

As a Case Manager at The Bridge, you will play a vital role in supporting formerly homeless adults with serious mental illness and/or substance use disorders.

These individuals reside in independent apartments, either within a single-site location or scattered throughout the community.

Your primary responsibility will be to deliver essential supportive services to help these individuals maintain stability and achieve their personal goals.

Key Responsibilities:

  • Provide case management and interventions to high-risk clients, including conducting assessments and facilitating referrals.
  • Assist in identifying strengths, needs, abilities, and goals, and create related Support Plans in the electronic client records system.
  • Use Motivational Interviewing techniques to help clients develop skills needed to maintain housing.
  • Assist and encourage individuals to develop natural supports and include these supports in Support Plans and client-related meetings.
  • Timely completion of required documentation for every client contact as per agency policy.
  • Co-facilitate groups in areas including wellness, leadership, housing, stability, employment, education, relationships, and healthy decision-making.
  • Provide case management services, including assistance with housing applications, referrals to medical and mental health care, maintaining active benefits, escorting, compliance with service providers, and coordinating care with outside providers.
  • Attend case conferences with the High Risk Coordinator to formulate interventions.
  • Attend staff meetings, supervision, and trainings as required.

Requirements:

  • Bachelor's Degree required.
  • Experience working with mental health, crisis intervention, and managing trauma-triggered behavior.
  • Excellent time management and organizational skills.
  • Team player who is stress-resistant, flexible, creative, and open to feedback.
  • Willingness to perform CPR/First Aid/AED/Naloxone.
  • Preferred knowledge of AWARDS.
  • Bilingual in Spanish is a plus.
  • Veterans or direct experience working with Veterans is preferred.
