Position Overview:

We are seeking a dedicated Registered Nurse Case Manager to join our team in the Utilization Management department. This role is vital in ensuring that our patients receive the highest quality of care through effective coordination and management of their healthcare plans.

Key Responsibilities:

  • Coordinate comprehensive healthcare plans for a designated patient population, utilizing care plans, critical pathways, and managed care protocols.
  • Collaborate with healthcare professionals to evaluate and enhance the quality, accessibility, and cost-effectiveness of care.
  • Maintain continuity of care by integrating all phases of patient management, ensuring that care is tailored to meet individual needs.
  • Provide exceptional patient care while considering age-specific, developmental, cultural, and spiritual requirements.
  • Assist in various clinical settings, including Emergency, Inpatient Rehabilitation, and Inpatient services.

Shift Availability: Days and Evenings

Status: Part time (24 hours per pay period)

Requirement: One weekend per month and one holiday per year

Qualifications:

  • Education: Bachelor’s Degree in Nursing is mandatory.
  • Experience: A minimum of three to five years of relevant clinical experience is required.
  • Licensure: Must hold a current Registered Nurse License in the State of Michigan. Certification in Case Management (CCM) is required within two years of eligibility.
