Position Overview:
We are seeking a dedicated Registered Nurse Case Manager to enhance our mission and serve as a compassionate presence in our community. This role is vital in ensuring the delivery of high-quality healthcare services.
Department: Utilization Management
Location: Chelsea, MI
Role Responsibilities:
The case manager will be responsible for:
- Coordinating healthcare plans for assigned patient populations.
- Utilizing care plans, critical pathways, managed care, and utilization review processes.
- Collaborating with all members of the healthcare team to evaluate care based on quality, access, and cost-effectiveness.
- Maintaining a continuum of care through the integration of all phases of patient management.
- Providing patient care that respects age-specific, developmental, cultural, and spiritual needs.
- Assisting with physicians' groups and being cross-trained in Emergency, Inpatient Rehab, and Inpatient settings.
Available Shifts: Days and Evenings
Part-Time Status: 24 hours per pay period (every two weeks)
Requirement: One weekend per month and one holiday per year
Qualifications:
- Education: Bachelor’s Degree in Nursing required.
- Experience: Three to five years of relevant experience with a solid understanding of the patient population.
- Licensure: Current Registered Nurse License in the State of Michigan; CCM certification required within two calendar years of eligibility.
