Job Description

We are seeking a dedicated Case Management Specialist to provide support services to individuals in need of housing and employment assistance.

About the Role

  • Identify customers in need of case management and connect them with local resources, including food, housing, clothing, and job training programs.
  • Collaborate with community agencies, healthcare providers, and other stakeholders to ensure seamless service delivery.
  • Develop and implement individualized service plans, monitoring progress and outcomes.
  • Maintain accurate records of customer interactions, progress, and outcomes.

Requirements

  • Bachelor's degree from an accredited institution required; experience can be substituted on a year-by-year basis.
  • Two years of direct client service experience working with populations experiencing homelessness and/or substance abuse.
  • Spanish language proficiency (written and verbal) required.
  • CT Community Health Worker certification or ability to obtain within six months.

Salary & Benefits

$22.34-$26.14 per hour, paid vacation days, sick leave, personal days, medical, dental, vision, 401K with employer match, and employee assistance program.
