We are seeking an experienced Case Manager and Healthcare Coordinator to join our team at TotalMed Staffing. This is a unique opportunity for a skilled professional to make a meaningful difference in the lives of patients and families.

About this Role

In this role, you will be responsible for managing and directing the use of resources to achieve the highest quality outcomes and patient experiences. You will coordinate and advocate for patients during hospitalization, from admission to post-discharge. Your key responsibilities will include monitoring the discharge plan implementation, identifying and addressing psychosocial and support-systems issues, overseeing the coordination of services, and ensuring cost-effective treatment while following patient's-rights regulations.

Job Requirements

To succeed in this role, you will need to have a strong background in case management nursing with a minimum of 1 year of experience. A current RN license is required, and BSN is preferred. Additionally, you should hold either CCM or ACM certification. Excellent verbal and written communication skills are essential for this position.

Compensation
