Job Title: Senior Case Manager

We are seeking a highly skilled and dedicated Senior Case Manager to join our team at Community Renewal Team (CRT). As a Senior Case Manager, you will be responsible for providing case management services to customers, including coordination of supervision, intake, assessments, referral, and discharge of customers. You will also be responsible for monitoring the integrity of data entered in Agency's STEPS and Funder data collection systems and compiling required reports.

About the Job

The estimated salary for this position is $60,000 - $80,000 per year, depending on experience.

You will work with customers to assess their needs and develop Individualized Service Plans (ISPs) that incorporate all special needs. You will also provide information and referral services, document outcomes resulting from such referrals, and facilitate customer access to community resources.

As a Senior Case Manager, you will serve as a liaison to community organizations to reach new or underserved customers and increase customer access to needed services. You will represent CRT at community meetings and outreach events, and maintain relationships with other community action agencies.
