Care Management Coordinator

We are seeking a highly skilled Care Management Coordinator to join our team at MJHS. As a key member of our healthcare team, you will play a vital role in ensuring high-quality, cost-effective care and services for our Elderplan members.
Responsibilities- Support professional Care Management and/or Clinical Service activities to ensure seamless care coordination for our Medicare, Medicaid, and/or dual product members.
- Collaborate with healthcare professionals to develop and implement effective care plans that meet the unique needs of our members.
- Utilize your expertise in utilization management and case management to ensure compliance with departmental and regulatory requirements.
- High School Diploma or equivalent; college degree preferred.
- One year prior managed care experience required.
- Prior experience in a healthcare setting preferred.
