Job Summary

We are seeking a highly skilled Care Management Specialist to join our team at MJHS Medical Associates, P.C. This role will be responsible for ensuring high-quality, cost-effective care and services for our Elderplan members through support of professional Care Management and/or Clinical Service activities.

Key Responsibilities
  • Support all aspects of care coordination for our ISNP, IESNP, and Elderplan Plus members in compliance with all departmental and regulatory requirements.
  • Receive and process assignments from ISNP Case Managers and Community staff/PCP outreach.
  • Collaborate with healthcare professionals to develop and implement individualized care plans.
  • Monitor and evaluate the effectiveness of care plans and make adjustments as needed.
Requirements
  • High School Diploma or equivalent; College Degree preferred.
  • One-year prior managed care experience preferred.
  • Prior experience in a healthcare setting preferred.
  • Familiarity with utilization management/case management.
