Job Title: Enrollment Specialist

At the Center for Human Development, we are seeking an experienced Enrollment Specialist to join our team. As an Enrollment Specialist, you will play a critical role in ensuring that enrollees are assigned a Care Coordinator within five business days.

Key Responsibilities:
  • Verify enrollee eligibility for CP services with MassHealth.
  • Utilize various avenues to obtain updated or confirm contact information for enrollees.
  • Initiate recruitment contact with enrollees through phone calls, text/emails, mailing letters, or conducting outreach visits.
  • Introduce enrollees to the ICP program and provide them with a handbook to review and sign participation forms.
  • Assign enrollees to appropriate teams and care coordinators using the enrollment care coordinator availability list.
  • Schedule initial visits for Care Coordinators.
Requirements:
  • Outreach to various communities throughout Western Mass.
  • Connect with different agencies to assist with enrollment.
  • Engage enrollees assigned to ICP.
  • Knowledge of MassHealth's Community Partners program and ability to discuss it with enrollees and community members.
  • Ability to work with people from diverse backgrounds.
