Home Health Physical Therapist

We are seeking a skilled Physical Therapist to join our team at Parx Home Health Care. As a Physical Therapist, you will play a vital role in providing high-quality care to patients in their homes and communities.
Key Responsibilities:- Conduct comprehensive physical therapy evaluations to identify patient needs and create personalized treatment plans.
- Teach patients and their families how to perform exercises and activities to promote recovery and improve mobility.
- Document patient progress and communicate updates to other healthcare professionals.
- Coordinate care with other medical professionals to ensure effective treatment delivery.
- Adhere to established policies and procedures, including infection control and safety protocols.
- Educate patients and their families on injury prevention and self-care techniques.
- Graduate from an accredited physical therapy program with a valid Physical Therapy license in the state of Florida.
- Minimum of one year of experience in physical therapy, preferably in a home health care setting.
- Demonstrated ability to create effective physical therapy treatment plans.
- Effective communication skills to communicate with patients, families, and other healthcare professionals.
- Ability to work independently and collaboratively as part of a larger healthcare team.
