About the Role

We are seeking a skilled Physical Therapist to join our team at Parx Home Health Care. As a Physical Therapist, you will play a vital role in providing high-quality care to patients in their homes and communities.

Key Responsibilities
  • Conduct comprehensive physical therapy evaluations to identify patient needs and create personalized treatment plans.
  • Develop and implement effective treatment plans to help patients achieve their goals and improve mobility.
  • Communicate with patients, families, and other healthcare professionals to ensure seamless care coordination.
  • Document patient progress and maintain accurate records in accordance with state and national regulations.
  • Collaborate with other medical professionals to ensure the most effective treatment is being delivered.
Requirements
  • Graduate from an accredited physical therapy program with a valid Physical Therapy license in the state of Florida.
  • Minimum of one year of experience in physical therapy, preferably in a home health care setting.
  • Demonstrated ability to create effective physical therapy treatment plans that promote patient recovery.
  • Effective communication skills, both verbally and in writing, to effectively communicate with patients, families, and other healthcare professionals.
  • Ability to work independently and collaboratively as part of a larger healthcare team.
