About the Role

We are seeking a highly skilled Registered Nurse to join our team as a Mental Health Nurse Specialist in Home Health. As a valued member of our team, you will play a crucial role in providing high-quality care to patients in their homes.

Job Summary

In this rewarding role, you will work closely with our interdisciplinary team to deliver comprehensive nursing care to patients with mental health needs. You will assess patients' needs, develop personalized care plans, and collaborate with healthcare professionals to ensure seamless patient care. Your expertise and compassion will make a significant difference in the lives of our patients and their families.

Responsibilities
  • Conduct initial and ongoing assessments of patients' mental health needs and develop targeted care plans.
  • Collaborate with physicians, social workers, and other healthcare professionals to ensure comprehensive patient care.
  • Provide education and support to patients and their families on mental health management and wellness strategies.
  • Maintain accurate and timely documentation of patient interactions and progress.
  • Participate in quality improvement initiatives to enhance patient outcomes and care delivery.
Requirements
  • Current RN licensure and current CPR certification.
  • A minimum of 5 years of experience as a Registered Nurse, preferably in home health or mental health settings.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and healthcare teams.
  • Strong assessment and problem-solving skills, with the ability to prioritize tasks and manage multiple responsibilities.
