Job Summary

We are seeking an experienced and compassionate RN Case Manager to join our team in the Plymouth area.

The successful candidate will provide direct patient care in the patient's home, conducting assessments, developing care plans, and working collaboratively with the patient's physician to provide optimum care.

Responsibilities
  • Provide direct patient care in the patient's home
  • Conduct assessments and develop care plans
  • Work collaboratively with the patient's physician to provide optimum care
  • Coordinate care and identify the need for other services in the home
Requirements
  • Current Massachusetts Nursing license in good standing
  • Registered Nurse (bachelors degree preferred)
  • 2 years of nursing experience required
  • Strong clinical assessment skills and the ability to prioritize and triage patients
  • Exceptional communication skills and the ability to work within a team structure
  • Excellent organizational skills and time management skills
  • Ability to work independently and understanding of how to access resources for patients
  • Strong computer skills

Please note that full benefits are available for positions with 30 hours, limited benefits are available for positions with 20 hours, and per diem roles do not offer benefits.
