Hospice Care Coordinator

We are seeking a skilled Registered Nurse to join our team as a Case Manager. This is a full-time position offering excellent benefits and competitive salary.

Job Responsibilities:
- Coordinate and oversee all aspects of patients' home care
- Collaborate with patient care teams to develop and implement care plans
- Educate families on patient condition, needs, and care
- Perform head-to-toe physical assessments
- Check vital signs and manage medications
- Administer and maintain catheters
- Complete documentation timely and accurately
- Supervise HHAs
- Regularly communicate patient progress to the clinical manager, physician, and care team
Salary and Benefits:
- $85,000 - $110,000 per year
- Medical insurance through Blue Cross & Blue Shield

- Mileage reimbursement of 50 cents/mile
