Job Overview:

About the Role:
This position involves working with patients and their families to provide emotional support and guidance during challenging times. As a Hospice Clinical Social Worker, you will assess patients' needs, develop care plans, and collaborate with healthcare teams to ensure high-quality care.

Qualifications:
To be successful in this role, you must possess a Master's Degree from an accredited school of Social Work or a related field and hold a valid license/registration to practice in the state(s) of agency operation. Medicare/Medicaid/Insurance specialty is preferred.
