Job SummaryWe are seeking a skilled Licensed Vocational Nurse (LVN) to join our team as a Home Health Care Specialist. Key Responsibilities- Collaborate with the Registered Nurse (RN) to develop and implement patient-specific care plans.
- Deliver high-quality skilled care to patients, including wound care, medication management, and vital sign monitoring.
- Document patient care delivery accurately and thoroughly in our electronic medical record (EMR) system.
- Contribute to a culture of caring through individual accountability and teamwork.
- Current Vocational/Practical Nurse License, valid state driver's license, and auto liability insurance.
- One year of experience as a Licensed Vocational/Practical Nurse in a clinical care setting, with home health experience preferred.
