* Collaborate with patients, families, and healthcare providers to ensure effective communication and coordination of care

* Monitor patient progress and adjust care plans as needed
* Participate in interdisciplinary team meetings to discuss patient care and develop strategies for improvement

Requirements:

* Current RN licensure in Texas or eligibility for licensure
* At least one year of experience as a RN in a clinical role
* Excellent communication and interpersonal skills
* Ability to work independently and as part of a team
* Strong problem-solving and critical thinking skills
