We are seeking a compassionate and skilled Registered Nurse Case Manager to join our team at Good Shepherd Hospice. As a RN Case Manager, you will play a vital role in providing high-quality care to patients and their families in our community.

Key Responsibilities
  • Conduct thorough nursing assessments to identify patient needs and develop individualized care plans.
  • Collaborate with attending physicians, hospice staff, and other healthcare professionals to ensure seamless care coordination.
  • Maintain open and empathetic communication with patients and families, addressing their concerns and providing emotional support.
  • Document patient progress, including medical history, treatment plans, and outcomes, in accordance with Good Shepherd Hospice policies and procedures.
  • Provide education and guidance to patients and caregivers on medication management, personal care, and safety protocols.
  • Participate in interdisciplinary team meetings and patient conferences to discuss care plans and address any concerns.
  • Ensure timely and accurate completion of paperwork, including Medicare and Good Shepherd Hospice forms.
  • Contribute to the development and implementation of hospice care policies and procedures.
Requirements
  • Current licensure as a Registered Nurse in the state where care is provided.
  • Previous experience in hospice, home health, or geriatrics preferred.
  • Strong organizational and self-management skills, with ability to prioritize tasks and manage multiple responsibilities.
  • Excellent communication and interpersonal skills, with ability to work effectively with patients, families, and healthcare professionals.
  • Commitment to the hospice philosophy of care and ability to provide compassionate, patient-centered care.
  • Valid driver's license and reliable transportation.
