Key Responsibilities- Conduct thorough nursing assessments to identify patient needs and develop individualized care plans.
- Collaborate with attending physicians, hospice staff, and other healthcare professionals to ensure seamless care coordination.
- Maintain open and empathetic communication with patients and families, addressing their concerns and providing emotional support.
- Document patient progress, including medical history, treatment plans, and outcomes, in accordance with Good Shepherd Hospice policies and procedures.
- Provide education and guidance to patients and caregivers on medication management, personal care, and safety protocols.
- Participate in interdisciplinary team meetings and patient conferences to discuss care plans and address any concerns.
- Ensure timely and accurate completion of paperwork, including Medicare and Good Shepherd Hospice forms.
- Contribute to the development and implementation of hospice care policies and procedures.
- Current licensure as a Registered Nurse in the state where care is provided.
- Previous experience in hospice, home health, or geriatrics preferred.
- Strong organizational and self-management skills, with ability to prioritize tasks and manage multiple responsibilities.
- Excellent communication and interpersonal skills, with ability to work effectively with patients, families, and healthcare professionals.
- Commitment to the hospice philosophy of care and ability to provide compassionate, patient-centered care.
- Valid driver's license and reliable transportation.
