Family Support Specialist

As a member of our team, you will have the opportunity to significantly influence the lives of numerous families across Indiana.
We are currently seeking a dedicated Home-based Case Manager to serve various counties, including Wells, Adams, Grant, Blackford, Jay, Delaware, and Randolph. In this role, you will deliver essential services directly within the homes of families, assisting them in reaching their objectives. Your responsibilities will include providing education, support, and transportation to families, as well as advocating for them in court and during crises to stabilize family dynamics. This position is primarily community-based.
Your Responsibilities:- Family Progress Monitoring and Coaching (50%)
- Collaboratively coach families, balancing education and service.
- Conduct regular visits to each family, including spontaneous check-ins.
- Assist families in achieving their personal goals.
- Offer educational resources and support to families.
- Provide necessary transportation for families.
- Accompany clients to court and provide informed recommendations.
- Engage with schools and various institutions regarding family matters.
- Respond to crisis situations and manage them effectively.
- Service Plans and Goal Development (20%)
- Create and update individualized service plans for each family.
- Establish and nurture relationships with community agencies.
- Collaborate with other social service organizations.
- Intake and Needs Assessment (15%)
- Identify crisis situations through assessments.
- Gather necessary information from families to complete assessments.
- Adhere to professional standards set by Firefly Children & Family Alliance.
- Reporting (15%)
- Maintain accurate case documentation and agency records.
- Prepare timely reports for relevant authorities.
- A Bachelor’s degree in social work or a related field is essential.
- Applicants must be at least 21 years old.
- A valid driver’s license and insurance with a clean driving record are required.
- Willingness to travel between client appointments.
- Familiarity with social service systems in the state is necessary.
- Understanding of confidentiality practices is crucial.
- Ability to respond to and appreciate cultural differences.
- Capability to develop relevant professional networks.
