We are actively looking for a full-time Client Support Specialist II for our Foster Hope initiative, which serves the surrounding community. This role offers a meaningful opportunity to positively influence the lives of others. The position requires travel up to 60% of the time.

Primary Responsibilities:

  • Conduct interviews, provide counseling, and resolve client issues related to social, emotional, financial, marital, health, or other challenges.
  • Travel 60% of the time to various counties to deliver services.
  • Provide Case Management Services, including assessment, development, and execution of case management strategies.
  • Organize and facilitate client support groups.
  • Establish and maintain collaborative relationships with medical personnel and representatives from various agencies and institutions.
  • Encourage clients towards independent living and enhance their self-esteem.
  • Prepare and maintain comprehensive case records, reports, and documentation, including follow-up and closure of cases.
  • Adhere to Meridian Health Services' standards, policies, procedures, and regulations in compliance with local, state, and federal laws and ethical professional standards.
  • Meet assigned productivity benchmarks.

Ideal Candidates Will Possess:

  • A Bachelor’s degree (BA, BS, BSW) in relevant fields such as Social Work, Sociology, Psychology, Criminal Justice, Education, Nursing, Recreational Therapy, Rehabilitation, Music Therapy, Art Therapy, Pastoral Counseling, or Child Development.
  • A minimum of five (5) years of experience in the Department of Child Services (DCS), Licensed Child Placing Agency (LCPA), or a related area is preferred.
  • A physical examination is required.
  • A valid driver's license with a driving record that complies with Meridian's driving policy, along with reliable transportation and proof of auto insurance.
  • A strong commitment to providing personalized support to help clients achieve mental, emotional, and physical wellness.
