Job Description

Job Summary:

Firefly Children and Family Alliance is seeking a dedicated and compassionate Case Manager - Family Support Specialist to join our team. As a key member of our organization, you will play a vital role in providing essential services to vulnerable children and families in our community.

Key Responsibilities:

  • Family Progress - Monitoring and Coaching (50%):
    • Collaborate with families to set and achieve goals, providing education and support to promote positive outcomes.
    • Conduct regular home visits to assess family needs and provide coaching to enhance family functioning.
    • Develop and implement service plans to address family goals and objectives.
    • Provide transportation and other necessary support to ensure family stability.
    • Attend court proceedings and provide recommendations to support family well-being.
    • Communicate effectively with schools, social services, and other community agencies to ensure comprehensive support.
    • Respond to crisis situations and provide emergency support to families in need.
  • Service Plans / Goal Development (20%):
    • Develop and update service plans to reflect changing family needs and goals.
    • Build relationships with community agencies and organizations to ensure access to resources and services.
    • Collaborate with other social workers and professionals to ensure comprehensive support for families.
  • Intake and Needs Assessment (15%):
    • Conduct thorough intake assessments to identify family needs and develop service plans.
    • Visit families in their homes to gather information and assess their needs.
    • Ensure that all assessments and service plans are completed in accordance with Firefly Children and Family Alliance standards.
  • Reporting and Documentation (15%):
    • Maintain accurate and timely records of family interactions, service plans, and progress.
    • Prepare written reports for the Department of Child Services and other stakeholders as required.

Requirements:

  • Bachelor's degree in social work or a related field.
  • At least 21 years of age.
  • Valid driver's license and insurance with no more than 6 points on driving record.
  • Ability to travel between appointments to serve clients in their community.
  • Knowledge of social service systems and confidentiality practices.
  • Appreciation for and ability to respond to cultural differences.
  • Strong communication and interpersonal skills.
