We are seeking a dedicated Home-Based Case Manager to serve various counties in Indiana. In this role, you will engage with families in their own environments, assisting them in reaching their objectives. Your responsibilities will include providing education, support, and transportation, as well as attending court sessions and responding to crises to help stabilize family dynamics. This is a field-based position, requiring active community involvement.

Your Responsibilities:
  • Family Progress Monitoring and Coaching (50%)
    • Collaboratively coach families, balancing teaching with service
    • Conduct regular visits to families, including unannounced check-ins
    • Assist families in achieving their set goals
    • Offer educational resources and support
    • Provide necessary transportation for families
    • Accompany clients to court and provide informed recommendations
    • Communicate with educational institutions and various agencies regarding family matters
    • Respond to crisis situations effectively
  • Service Plans and Goal Development (20%)
    • Create individualized plans with specific goals for each family
    • Regularly update these plans and goals
    • Establish relationships with community agencies
    • Collaborate with other social service organizations
  • Intake and Needs Assessment (15%)
    • Identify crisis situations
    • Conduct home visits to gather essential information for assessments
    • Adhere to professional standards set by Firefly Children & Family Alliance
  • Reporting (15%)
    • Maintain accurate case documentation and agency records
    • Prepare timely reports for relevant authorities
Qualifications:
  • Bachelor's degree in social work or a related field is required.
  • Must be at least 21 years of age.
  • A valid driver's license and insurance with a clean driving record.
  • Ability to travel between client appointments.
  • Familiarity with social service systems in the state.
  • Understanding of confidentiality practices.
  • Ability to appreciate and respond to cultural differences.
  • Capacity to develop a network of relevant contacts.
