Job Summary:

The MDS RN Coordinator will be responsible for:

  • Verifying documentation of all nursing care administered
  • Collecting and documenting resident MDS data
  • Completing resident assessments and care plans
  • Collaborating with the interdisciplinary team to discuss resident plans of care
Requirements:

To be successful in this role, you will need:

  • A Registered Nurse (RN) license in good standing in Illinois
  • Minimum 3 years of relevant experience
  • Previous MDS/Long Term Care experience preferred
  • Familiarity with PDPM requirements
  • Excellent supervisory and communication skills
