Position Overview
In the role of Hospice Aide, you will deliver personal care, supportive services, education, and assist with light household tasks as outlined in a care plan established by the Registered Nurse Case Manager. You will also be responsible for communicating and documenting any changes in the patient's condition to ensure a cohesive approach with the entire Inter-Disciplinary Team.
Key Responsibilities
- Ensure patients receive the necessary attention to maintain a clean and organized environment.
- Assist with personal hygiene tasks, including bathing, skin care, oral hygiene, and toileting.
- Report identified needs and potential solutions to the interdisciplinary team.
- Measure and record vital signs.
- Engage in staff meetings, educational programs, and community outreach activities.
- Strong communication and interpersonal skills.
- Flexibility to work various shifts, including evenings and weekends as required.
- Ability to exercise sound judgment in following instructions.
- High level of integrity, particularly in maintaining confidentiality.
- Possession of a valid driver's license and auto liability insurance.
A high school diploma or equivalent; or one to three months of related experience and/or training; or a combination of education and experience.
State-specific certifications may be required.
