**Job Summary:**

We are seeking a skilled and compassionate RN to join our team as a Case Manager. In this role, you will be responsible for assessing patients' needs, developing and implementing individualized care plans, and coordinating with healthcare providers to ensure seamless care transitions.

**Responsibilities:**

* Conduct comprehensive assessments of patients' physical, emotional, and social needs
* Develop and implement individualized care plans that address patients' unique needs and goals
* Coordinate with healthcare providers, including physicians, therapists, and other healthcare professionals, to ensure timely and effective care
* Monitor patients' progress and adjust care plans as needed
* Collaborate with patients and their families to ensure they are informed and engaged in their care
* Maintain accurate and up-to-date records of patient interactions and care plans

**Requirements:**

* Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation
* One (1) year experience as a RN
* Required Certifications and Licensures: Licensed to practice as a registered nurse in the state of agency operation
* Must possess and maintain valid CPR certification while employed in a clinical role
* Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
