Care Coordinator for Adults with Developmental Disabilities

Future Transitions is seeking a dedicated Care Coordinator to provide direct care support to developmentally disabled adults in their home and community.
The purpose of this role is to schedule staff assigned to consumers, manage authorized hours, and support personal care, light housekeeping, recreational activities, transportation, meal preparation, community access, and other duties outlined in the Individual Service Plan (ISP).
Responsibilities- Schedule staff assigned to consumers
- Manage authorized hours for each consumer
- Provide personal care, light housekeeping, recreational activities, transportation, meal preparation, and community access
This position requires implementing the consumer's vision and choices as stated in the ISP. A valid California driver's license, current vehicle registration, and use of a vehicle in good running condition during shift hours are necessary. Additionally, valid automobile insurance for that vehicle and all passengers, with employee listed on the policy, is required.
Qualifications and Requirements- 1 year Supervisor Experience or Associates Degree
- Three (3) or more years of experience providing care for adults with developmental disabilities preferred
- Current CPR and First Aid Certification for adults and children
- Cell phone with data plan and app capabilities
- Successfully complete a Live Scan criminal background check through fingerprint clearance
We offer a competitive salary range of $19.00 - $20.00 hourly, reflecting our commitment to fair compensation and employee satisfaction.
