Job Summary

UAB Medicine - UA Health Services Foundation (UAHSF) is seeking a highly skilled Advanced Practice Provider (APP) to join our team in a primary care role for frail homebound adults. As a key member of our interdisciplinary team, you will provide medical care and treatment to patients in their homes within a 30-mile radius of UAB.

Key Responsibilities
  • Provide comprehensive medical care and treatment to patients in their homes, including performing physical examinations, interpreting medical records, and coordinating care with other healthcare providers.
  • Work autonomously and to the top of your licensure with the support of a collaborating physician.
  • Participate in performance improvement activities and contribute to the development of care plans and treatment protocols.
  • Collaborate with other healthcare professionals to provide high-quality, patient-centered care.
Requirements
  • Master's degree in Nursing with an Adult Primary Care NP or Family NP certification, or graduate from an accredited Physician Assistant program.
  • Currently certified as an Advanced Practice Nurse (APN) with Certified Registered Nurse Practitioner (CRNP) Specialty by the Alabama Board of Nursing or licensed as a Physician Assistant by the Alabama Board of Medical Examiners with certification as a Physician Assistant (PA-C) by the National Commission on Certification of Physician Assistants.
  • Certified in Basic Life Support (BLS) from the American Heart Association (AHA) is required or should be obtained within 30 days of employment.
Preferred Qualifications
  • 1-2 years of APP experience is strongly preferred.
