Job Summary

Carespring is seeking a highly skilled Admission Coordinator RN to join our team. As an Admission Coordinator RN, you will be responsible for assessing new and existing patients to ensure all care needs are addressed and documented on the MDS.

Key Responsibilities
  • Oversee the MDS process, adhering to the RAI manual.
  • Collaborate with multiple MDS nurses to complete initial assessments for new admissions.
  • Work under the supervision of the DON to ensure MDS nurses properly manage patient admissions, MDS assessment, and care planning completion.
  • Submit required documentation weekly or as designated by the Director of Nursing, reviewing validation reports for accuracy.
  • Maintain the ability to obtain and review QM's, working with the IDT to initiate improvement projects to enhance resident outcomes.
  • Attend IDT Medicare and Medicaid meetings to collaborate on the MDS process.
  • Develop and update comprehensive care plans and nurse aide care plans (Kardex) in conjunction with the MDS to accurately address resident needs.
  • Oversee non-nursing sections of the MDS and CAA's to ensure accuracy.
  • Coordinate, manage, and monitor care plans, identifying problems and needs of residents by reviewing order recap reports and nurses' notes.
  • Monitor residents for changes in status to determine the need for significant changes per RAI.
  • Ensure supportive documentation is accurate according to MDS/RAI regulations.
  • Work with facility consultants as necessary, implementing recommended changes as required.
  • Complete required paperwork, including AIMS, Fall Risk scale, Braden, Progress note, Care Plan, and nurse aide care plan (Kardex).
Requirements
  • Current RN license for the state in which the facility is located.
  • Flexibility with working schedule and ability to work independently.
  • Understanding of the MDS process and willingness to continue learning and remaining current with MDS updates set by CMS.
  • Preferred knowledge of PDPM (skilled reimbursement) and RUG (Long Term Care reimbursement).
