Position Overview

The Care Coordinator plays a vital role within our Medication Assistance Program and Outpatient Services team at The Crossroads Center. This position is dedicated to providing essential case management and clinical support to women in Ohio who are facing challenges related to substance use and mental health disorders. The Care Coordinator is instrumental in promoting positive treatment outcomes through direct assistance, collaboration, and connection to vital resources.

Core Responsibilities
  • Deliver therapeutic and case management services to adults experiencing psychiatric disabilities, enhancing their community engagement.
  • Conduct outreach efforts to engage potential clients in various settings, including correctional facilities and healthcare institutions.
  • Perform thorough assessments to determine diagnoses, functional levels, and support systems.
  • Assist clients in establishing objectives and formulating Coordinated Care Plans, ensuring they are updated annually.
  • Guide clients in applying for and securing essential benefits, such as SSI, SSDI, Medicaid, and Medicare.
  • Facilitate access to local services, including medical care, housing assistance, rehabilitation programs, and social support.
  • Reconnect with clients who have lost contact, ensuring they remain engaged with necessary services.
  • Provide therapeutic interventions, including individual, group, and crisis counseling.
  • Maintain precise documentation within the Electronic Health Record (EHR) system.
  • Achieve weekly, monthly, and quarterly billing objectives.
  • Collaborate with colleagues, participate in 24-hour on-call coverage, and support the daily operations of related programs.
Qualifications & Skills
  • Bachelor's degree in psychology, social work, or a related discipline (Master's degree preferred).
  • Certification as a Chemical Dependency Counselor Assistant (CDCA) in the State of Ohio.
  • A minimum of four years of experience working with individuals diagnosed with mental health conditions or related fields; trauma-informed training is advantageous.
  • Strong written and verbal communication abilities, with the capacity to engage effectively with clients, colleagues, and community partners.
  • Familiarity with DSM IV diagnoses and evidence-based practices, including Motivational Interviewing and trauma-informed care.
  • Ability to work autonomously as well as collaboratively within a team environment.
  • Exceptional organizational and time management skills.
  • A valid Ohio driver's license, reliable transportation, and current insurance are required.
Work Environment & Physical Demands

This role necessitates regular use of hands and fingers, along with effective communication skills. The Care Coordinator will frequently need to walk, balance, stoop, kneel, and drive. Occasionally, lifting up to 15 pounds may be required. The position may involve traveling to various locations and providing transportation for clients.
